9. IELTS/TOEFL/TSE Certificate/Score Report
10. NCLEX Certificate/Score Report
11. PART B of ETA 750
12. Experience letters from former employers
13. Current Employment Certificate
14. Reference / Recommendation Letters (if any)
15. Recent Resume
16. Birth Certificate
17. 2 Passport size photograph (2X2)
18. Copies of passports Yourself, Spouse and Children.
For NY employment also required following at later stage.

2. NY State board of nursing application for first time registrants
3. NY State application for Limited Permit
